Consequence Record
A structured artifact that captures human judgment at the point of a consequential decision — before the outcome arrives.
The Gap
Systems record what was done. They rarely record what was held.
A person can approve an AI recommendation without the system capturing what they understood, where they saw the limits of the AI contribution, or what they committed to monitor or revisit.
What It Contains
Actor. Who held responsibility for this decision.
Consequence scope. What outcomes the actor accepted as theirs.
AI reliance boundary. Where AI input was trusted and where it was not.
Uncertainty posture. What was unknown and acknowledged.
Return commitment. When the actor expected to revisit.

Continuity
